I certify that the information provided on this RRHF Volunteer Form is true and correct to the best of my knowledge. I understand that, prior to being approved, this information will be verified. I authorize a RRHF Representative to contact my references as listed above.

I understand that I am required to sign a Release of Liability and Waiver before volunteering at an activity. If I so choose, I can be assigned a Mentor until I feel comfortable to do an activity on my own. Additionally, I agree to provide my own automobile and health insurance.

I understand that any and all services performed for Rottweiler Rescue Rehoming and foundation are on a volunteer basis and RRHF will not be billed at any rate for these services

I understand that all Rottweiler Rescue Rehoming and foundation dogs have received an initial physical examination and inoculations. A record of a RHR dog’s known medical history is available.

I agree to assume full responsibility for the care and management of any RRHF dog while it is under my care. RHR is not responsible for any actions of the RHR dog while in my care.

I agree not to visit the premises where a RRHF dog is housed or remove any RRHF dog at any time without written permission from the dog’s foster parent or a RRHF board member.

I agree to give my permission for an RRHF representative to remove a dog belonging to RHR from my premises and entry shall not constitute a trespass upon the premises occupied.

Release of Liability and Waiver

I agree to provide my own automobile and health insurance. I hereby release and agree to hold harmless Rottweiler Rescue Rehoming and foundation (RRHF) or any of its founders, officers, directors, and representatives of RRHF, for liability, loss, claim, and demands that may occur from loss, damage, injury, or death to my person or property, in any way resulting from, or arising in connection with, volunteering for Rottweiler Rescue Rehoming and foundation. I know the risk and danger to myself and my property while volunteering for Rottweiler Rescue Rehoming and foundation and this is done so voluntarily and in reliance upon my own judgment and ability. I thereby assume all risk of loss, damage, or injury (including death) to myself and my property from any cause whatsoever. In addition, I agree to not hold RRHF or any of its founders, officers, directors, volunteers, fosters, or RRHF representatives liable for any physical, emotional, or property damages that are a direct or indirect result of activities involved in the placement, transport, training, or evaluating of dog in any way associated with RRHF. This includes any and all activities I perform as a RRHF volunteer. This includes any and all activities I perform as a Rottweiler Rescue Rehoming and foundation volunteer. Authorization will expire five (5) years from today,
